MIDDLE SCHOOL ATHLETIC & HEALTH FORMS
MS Athletic Fee: The athletic fee for student participation will be $70 for the 2024-25 school year. This fee is a one-time fee for the school year that allows a student to participate in sports throughout the year. The fees collected are intended to help defray the operational cost of the middle school athletic budget.
Cape Elizabeth is transitioning to a new online payment system called Vanco. Instructions on how to set up your account can be found here. Payments can be made online for any fee on your student’s account via credit/debit card or eCheck/ACH payment. You can also continue to pay via check or cash in person. You can find your fees on the new 'Fees' tab in Powerschool. For assistance with online payments, please contact the Cape Elizabeth Technology Department at: techhelp@capeelizabethschools.org
The Cape Elizabeth School Department encourages participation in school activities regardless of financial status. To request a waiver (click here), If you have any questions please contact the Athletic Director, Jeff Thoreck at jthoreck@capeelizabethschools.org, or the Middle School Athletic Liaison, Charlie Tryder at ctryder@capeelizabethschools.org.
Health History Form - REQUIRED for all students, must be updated annually prior to the beginning of every school year. Please log in to your PowerSchool Family Portal to complete and submit electronically.
Physical Exam Form (Click Link to Open) - A physical exam record and/or letter of medical clearance must be received prior to the start of the athletic season and must include the date of the exam and/or date of clearance. Records may be sent by mail, fax, or email to CEMS or per the contact information in this letter.
Action Plans - All action plans must be updated and signed by your child's healthcare provider annually. Parent signature is also required if your child is self-carrying an inhaler or epinephrine auto-injector.
